Ультразвуковое исследование выполнялось на аппарате Epiq 7 (Philips, Нидерланды) с использованием ультразвукового контрастного препарата Соновью (Bracco Swiss S.A., Швейцария) и программного обеспечения QLAB (Philips, Нидерланды). Работа проходила в два этапа, в первый этап (количественный анализ) включены результаты исследования всех 93 пациентов, во второй этап (количественный анализ) - 50 пациентов (29 (58,0%) со злокачественными опухолями и 21 (42,0%) - с доброкачественными). При проведении качественного этапа статистический анализ проводился по стандартным параметрам, количественного - по стандартным параметрам (наклон при подъеме (AS (ascending slope)) (дБ/с), время до пика интенсивности (TTP (time to peak)) (с), пик интенсивности (PI (peak intensity)) (дБ)) и их производным (индекс AS (отношение AS в опухоли к AS в неизмененной паренхиме), индекс TTP (отношение TTP в опухоли к TTP в неизмененной паренхиме), индекс PI (отношение PI в опухоли к PI в неизмененной паренхиме)). Время вымывания ультразвукового контрастного препарата из злокачественной опухоли достоверно меньше, чем из доброкачественной опухоли (P = 0,001) и из прилежащей паренхимы (корковый слой) (P &lt; 0,0001) (качественный анализ). Индекс AS в злокачественной опухоли достоверно больше, чем в доброкачественной (P &lt; 0,000) (количественный анализ). Тест “время вымывания из опухоли ≤94 с - злокачественная опухоль почки” характеризуется AUC 0,749, чувствительностью 86,1%, специфичностью 61,9% (качественный анализ). Тест “индекс AS &gt; 0,81 - злокачественная опухоль почки” характеризуется AUC 1,000, чувствительностью 100,0%, специфичностью 100,0% (количественный анализ). После диагностики злокачественного образования почки при использовании порогового значения индекса AS 0,81 (1-й шаг) возможно прогнозирование степени G &gt; 1 (G2 и выше) с AUC 0,900, чувствительностью 85,0%, специфичностью 88,9% при пороговом значении индекса TTP 0,95 (≤0,95). Проведенная работа имеет ряд ограничений, что требует подтверждения результатов при проведении дальнейших исследований.</p></abstract><trans-abstract xml:lang="en"><p>The results of CEUS of the kidneys, performed in 93 patients with renal tumors (72 (77.4%) - malignant, 21 (22.6%) - benign), were analyzed. The examinations utilized Epiq 7 scanner (Philips, Netherlands) with QLAB software (Philips, Netherlands) and the Sonovue (Bracco Swiss SA, Switzerland) as ultrasound contrast agent. The qualitative CEUS analysis was performed in all 93 patients as the first step. The quantitative CEUS analysis was performed in 50 patients (29 (58.0%) with malignant tumors and 21 (42.0%) with benign ones) as the second step. Statistical analysis was carried out in accordance with standard qualitative parameters at the first stage and standard quantitative parameters (ascending slope (AS, dB/s), time to peak (TTP, s), peak intensity (PI, dB)) and their derivatives (indices), calculated as the ratio of tumor/normal parenchyma parameter value (AS index, TTP index, PI index) at the second step. Malignant tumor washout time was significantly shorter compared with the benign tumors (P = 0.001) and with the adjacent parenchyma (cortical layer) (P &lt; 0.0001) (qualitative analysis). Malignant tumors AS index was significantly higher than in the benign ones (P &lt; 0.000) (quantitative analysis). The test “tumor washout time ≤94 s - malignant renal tumor” was characterized by AUC 0.749, sensitivity 86.1%, specificity 61.9% (qualitative analysis). The test “AS index &gt; 0.81 - malignant renal tumor” was characterized by AUC 1.000, sensitivity 100.0%, specificity 100.0% (quantitative analysis). After diagnosis of malignant renal masses using AS index threshold value 0.81 (1st step), it is possible to predict the degree G &gt; 1 (G2 and higher) with a TTP index threshold value 0.95 (AUC - 0.900, sensitivity - 85.0%, specificity - 88.9%) (≤0.95) (2nd step).
