В статье представлено клиническое наблюдение инфекционного эндокардита у девушки 17 лет, диагноз которого был выставлен лишь спустя месяц после дебюта заболевания. Трансторакальная эхокардиография позволила выявить крупную вегетацию на задней створке митрального клапана. Спустя месяц массивной антибактериальной терапии при повторном эхокардиографическом исследовании вегетация не визуализировалась. Для уточнения состояния клапанного аппарата сердца больной выполнена трансэзофагеальная эхокардиография, позволившая выявить дефект митрального клапана - перфорацию его задней створки. Приведенный случай наглядно демонстрирует, что ошибочная диагностика или постановка запоздалого диагноза при инфекционном эндокардите сопряжена с развитием тяжелых осложнений со стороны клапанного аппарата сердца, в нашем случае - перфорации задней створки митрального клапана у молодой девушки. При лихорадках неясного генеза следует рассматривать вероятность инфекционного эндокардита во всех возрастных группах, даже у лиц, не имеющих очевидных факторов риска развития заболевания. Необходимо как можно раньше производить бактериологический посев крови, тщательно мониторировать аускультативную картину сердца и как можно раньше выполнить доступную и информативную трансторакальную эхокардиографию. При отрицательных результатах трансторакальной эхокрадиографии и высоком клиническом подозрении на инфекционный эндокардит следует воспользоваться трансэзофагеальным ультразвуковым датчиком.</p></abstract><trans-abstract xml:lang="en"><p>The article presents a clinical case of infective endocarditis in 17-year-old girl, the diagnosis of which was established only a month after disease debut. Transthoracic echocardiography revealed large vegetation on the posterior mitral valve leaflet. After a month of antibacterial therapy at repeated echocardiographic examination vegetation was not found. To clarify the patient’s heart valves state transesophageal echocardiography was performed. The transesophageal examination showed mitral valve defect-posterior leaflet perforation. The case clearly demonstrates that the erroneous or late infective endocarditis diagnosis is associated with the development of severe heart valves complications. In our case - posterior mitral valve leaflet perforation in a young girl. In the case of unknown genesis fevers, the probability of infective endocarditis should be considered in all age groups. It is necessary to perform transthoracic echocardiography as early as possible.
