Вычислительная векторэлектрокардиография (вЭКГ) является методом визуализации суммарного выброса вектора сердца в трехмерном пространстве на основе 12 отведенных и распределенных вычислительных процедур. Этот метод может быть сторонним способом оценки тяжести и прогноза у пациентов с острой декомпенсацией сердечной недостаточности (ОДСН).</p></sec><sec><title>Цель исследования</title><p>Цель исследования: изучить параметры вЭКГ, связанные с неблагоприятным прогнозом у пациентов с ОДСН.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проанализированы данные вЭКГ 100 пациентов с ОДСН. Всем пациентам проводилось исходно клиническое обследование, эхокардиография и определение натрийуретического пептида (NT-proBNP). В ходе наблюдения в течение 12 мес фиксировались летальные исходы. Оценивалась взаимосвязь показателей вЭКГ и NT-proBNP с прогнозом пациентов.</p></sec><sec><title>Результаты</title><p>Результаты. В данном исследовании скончались 17 пациентов. По полученным данным вЭКГ выжившие пациенты имели меньший пространственный угол QRS-T по сравнению с умершими (p = 0,039). Выявлены статистически незначимые показатели (p = 0,076) индекса планарности в группе скончавшихся пациентов. По данным ROC-анализа с чувствительностью 53% и специфичностью 71% было получено отрезное значение пространственного угла QRS-T, равное 164,5°. При превышении этого значения вероятность летального исхода увеличивается. Было выявлено пороговое значение неблагоприятного прогноза для NT-proBNP у пациентов с величиной угла QRS-T 164,5°, которое составило 999,35 пг/мл.</p></sec><sec><title>Заключение</title><p>Заключение. вЭКГ может быть использована в качестве простого метода для прогнозирования и оценки риска летального исхода у пациентов с ОДСН и сниженной фракцией выброса левого желудочка. Наиболее значимым показателем является значение пространственного угла QRS-T. Увеличение пространственного угла QRS-T связано с повышением риска летального исхода. Таким образом, анализ данных вЭКГ может быть полезным инструментом для идентификации пациентов с высоким риском и принятия решений по тактике ведения.</p></sec></abstract><trans-abstract xml:lang="en"><p>Computational vector electrocardiography (vECG) is a method for visualizing the total heart vector output in 3-dimensional mode based on 12 lead and distributed computational procedures. This method can be an additional to assess severity and prognosis in patients with acute decompensated heart failure (ADHF).</p><sec><title>Objective</title><p>Objective: to investigate the vECG parameters associated with poor prognosis in patients with ADHF.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. ECG data of 100 patients with ADHF were analyzed. All patients underwent baseline clinical examination, echocardiography, and a natriuretic peptide (NT-proBNP) test. Mortality was recorded during follow-up for 12 months. The correlation of vECG and NT-proBNP parameters with the prognosis of patients was evaluated.</p></sec><sec><title>Results</title><p>Results. Due to the period of follow-up, 17 patients died. According to vECG data, the surviving patients had a smaller spatial QRS-T angle compared to the deceased patients (p = 0.039). Statistically insignificant values (p = 0.076) of the planarity index in the group of deceased patients were revealed. ROC-analysis revealed the cut-off value of QRS-T spatial angle equal to 164.5°with sensitivity of 53% and specificity of 71%. The QRS-T spatial angle values above the cut-off value indicate a higher probability of lethal outcome. The cut-off value of 999.35 pg/ml of NT-proBNP in patients with the value of QRS-T angle of 164.5° associated with an unfavorable prognosis was revealed.</p></sec><sec><title>Conclusions</title><p>Conclusions. vECG can be used as a simple method to predict and assess the risk of fatal outcome in patients with ADHF and reduced left ventricular ejection fraction. The most significant parameter is the value of QRS-T spatial angle. An increase in the QRS-T spatial angle is associated with an increased risk of fatal outcome.
