Представлены два клинических наблюдения пациентов с меланомой кожи и метастатическим поражением регионарых ЛУ. Выполнено ультразвуковое исследование поверхностных ЛУ. Результаты ультразвукового исследования сопоставлены с данным рентгенологических (компьютерная томография с контрастным усилением) и радиологических методов (однофотонная эмиссионная компьютерная томография) и верифицированы данными гистологического исследования операционного материала.</p></sec><sec><title>Результаты</title><p>Результаты. Клиническое наблюдение № 1 демонстрирует классическую картину злокачественно измененных ЛУ при ультразвуковом исследовании: полициклический контур, отсутствие дифференцировки и периферический тип васкуляризации. Напротив, клиническое наблюдение № 2 представляет минимально выраженные ультразвуковые изменения: округлую форму, локальные утолщения коркового слоя и гипоэхогенные зоны в ЛУ с единичными локусами кровотока, которые также характеризуют вторичные изменения.</p></sec><sec><title>Заключение</title><p>Заключение. Ультразвуковое исследование позволяет по набору соответствующих признаков заподозрить наличие вторичных изменений в ЛУ при меланоме, что может быть использовано на первой линии диагностики для определения дальнейшего плана обследования и лечения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective. To demonstrate, using clinical cases, the diagnostic value and the role of ultrasound in assessing metastatic involvement of superficial regional lymph nodes (LN) in patients with cutaneous melanoma.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. Two clinical cases of patients with cutaneous melanoma and regional lymph nodes metastatic involvement are presented. Results of superficial lymph nodes ultrasound were compared with data of radiological methods (contrast-enhanced CT  and SPECT) and verified by histopathological examination of surgical specimens.</p></sec><sec><title>Results and Discussion</title><p>Results and Discussion. Case No.1 demonstrates the classic ultrasound features of lymph-node malignancy: a polycyclic outer contour, loss of corticomedullary differentiation, and a peripheral type of vascularization. In contrast, Case No. 2 shows poor ultrasound features: a rounded LN shape, focal cortical thickening, and hypoechoic areas with a few vascular loci, which are also indicative of secondary involvement.</p></sec><sec><title>Conclusion</title><p>Conclusion. Ultrasound allows to suspect the  secondary metastatic lymph nodes changes by the detection of a set of relevant signs in melanoma patients.
