Экстранодальная диффузная крупноклеточная В-клеточная лимфома – заболевание с первичным поражением любого органа, кроме лимфатических узлов. Нейролимфоматоз представляет собой процесс инфильтрации клетками лимфомы ткани периферических нервов, корешков и черепно-мозговых нервов. В рамках назначен ного неврологом ультразвукового исследования левого седалищного нерва проведено обследование пациентки 67 лет. Ультразвуковое исследование проводилось на аппарате Affinity 70 (Philips, Нидерланды) линейным датчиком частотой 4–18 МГц. При осмотре левого седалищного нерва на уровне средней трети бедра выявлено наличие интраневрального, занимающего всю площадь поперечного сечения нерва, бесструктурного веретенообразного образования резко сниженной эхогенности, неоднородной эхоструктуры, с линейными анэхогенными (жидкостными) прослойками. Размеры образования соста вили: длина (протяженность вдоль нерва) – 9,5 см, толщина – 2,3 см, ширина – 3,5 см. Площадь поперечного сечения – 6,5 см2. В режиме цветового допплеровского картирования в образовании определялись единичные сосудистые сигналы. Вокруг образования отмечалось повышение эхогенности, соответствующее десмопластической реакции. После проведения магнитно-резонансной томографии была подтверждена ультразвуковая находка в виде интраневрального образования. Открытая биопсия нерва показала наличие диффузной крупноклеточной В-кле точной лим фомы. Проведено химиотерапевтическое лечение по схеме R-CHOP, включающее 6 курсов, между которыми с целью динамического наблюдения за структурой и размерами образования выполнялись ультразвуковые исследования. Таким образом, на начальном этапе диаг ностического пути наиболее быстро получить информацию о наличии у пациента образования периферических нервов и предположить его природу можно с помощью ультразвукового исследования.</p></abstract><trans-abstract xml:lang="en"><p>Extranodal diffuse large B-cell lymphoma characterized by any organ primary lesion, except for lymph nodes. The article presents a rare case of this disease with primary site in the sciatic nerve. Neurolymphomatosis is a process peripheral nerves, roots, and cranial nerves infiltration by the lymphoma cells. Ultrasound examination of the left sciatic nerve was performed in 67-years-old female patient with the use of Affinity 70 (Philips, Netherlands) with 4–18 MHz linear probe. A local intraneural fusiform, markedly hypoechoic lesion (9.5 × 2.3 × 3.5 cm), covering the entire cross-sectional area of the nerve was found at the level of the middle third of the thigh. The loss of a typical nerve ultrasound pattern, heterogeneity of the structure with linear anechoic (liquid) layers, and single signals in color Doppler imaging were revealed. Focal hyperechogenicity of tissue, corresponding to a desmoplastic reaction was found on the periphery of the lesion. Magnetic resonance imaging confirmed the presence of the intraneural mass. An open nerve biopsy showed the presence of diffuse large B-cell lymphoma. Six cycles of R-CHOP chemotherapy were performed with dynamic ultrasound assessment of the size and structure of the lesion in the intervals between them.
