Обследовано 63 пациента (36 мужчин и 27 женщин) в возрасте от 31 до 79 лет. В структуре заболеваемости 6 случаев ревматического митрального стеноза, 10 - соединительнотканной дисплазии и отрыва хорд митрального клапана, 21 - аортального стеноза различной этиологии, 18 - аневризмы восходящего отдела аорты и аортальной недостаточности, 6 - ишемической болезни сердца и 2 - объемного образования левого предсердия. Трехмерная эхокардиография при исследовании из средней трети пищевода имеет диагностические преимущества для оценки анатомических особенностей устьев легочных вен, ушка левого предсердия и межпредсердной перегородки по сравнению с двумерной эхокардиографией. Оценка размеров и объемов левого предсердия при чреспищеводной эхокардиографии в двумерном и трехмерном режимах более корректна при трансгастральном доступе. При сравнении объемов и фракций выброса левого предсердия при двумерной и трехмерной чреспищеводной эхокардиографии выявлены достоверные различия значений активного ударного объема левого предсердия, что, возможно, связано с завышением полученных параметров в двумерном режиме.</p></abstract><trans-abstract xml:lang="en"><p>Left atrium and pulmonary veins examination using two- and three-dimensional real-time transesophageal echocardiography was performed in 63 cardiac surgery patients (36 men and 27 women) aged from 31 up to 79 years old. There were 6 cases of rheumatic mitral stenosis, 10 - connective tissue dysplasia and mitral valve chordae rupture, 21 - aortic stenosis of different etiology, 18 - aortic ascending aneurysms and aortic insufficiency, 6 - ischemic heart disease, and 2 - left atrial mass. There were advantages of three-dimensional echocardiography performed from middle part of esophagus in comparison with two-dimensional echocardiography. It was useful for assessment of pulmonary veins orifices anatomy, left atrial appendage, and intraventricular septum. Transgastric access during transesophageal echocardiography using two- and three-dimensional modes was more accurate for left atrial size and volume assessment.
