Проведено ультразвуковое исследование сердца и сосудов 355 пациентов с хронической болезнью почек (155 пациентов с хронической болезнью почек 1-5-й стадии и 200 пациентов с хронической болезнью почек 5Д стадии, получающих лечение диализом). Отмечена высокая распространенность кальциноза структур сердца и стенок артерий у пациентов с хронической болезнью почек, достигающая наибольшей выраженности у пациентов с терминальной почечной недостаточностью. В додиализной группе преобладающим вариантом был изолированный кальциноз аортального клапана; у пациентов, получающих заместительную почечную терапию, значительно чаще встречался сочетанный кальциноз аортального и митрального клапанов. Пошаговый многофакторный регрессионный анализ показал в качестве независимых факторов развития кальциноза структур сердца возраст пациентов (Р &lt; 0,001), длительность заместительной почечной терапии (Р = 0,002), величину кальциево-фосфорного произведения (Р &lt; 0,001) и уровень паратиреоидного гормона (Р = 0,002). Отмечено прогрессирующее снижение эластических свойств артериальной стенки у пациентов с хронической болезнью почек. По данным множественного пошагового регрессионного анализа независимыми факторами риска, оказывающими влияние на развитие кальциноза стенок артерий, были возраст (Р &lt; 0,001), уровень фосфора сыворотки крови (Р &lt; 0,001), наличие сахарного диабета (Р = 0,005). При многофакторном регрессионном анализе независимыми факторами, ассоциированными с увеличением смертности, были скорость распространения пульсовой волны (Р = 0,003); деформация, которую испытывает стенка артерии за сердечный цикл (CS) (Р = 0,014); кальциноз структур сердца (Р = 0,038).</p></abstract><trans-abstract xml:lang="en"><p>Aim of the study was to assess cardiovascular system changes according to the ultrasound data in patients with chronic kidney disease and their relationship with mineral and bone disorder. 355 patients with chronic kidney disease underwent ultrasound investigation of the cardiovascular system (155 patients with chronic kidney disease of 1-5 stage (1st group) and 200 patients - of 5D stage (undergoing dialysis) (2nd group)). There was a high prevalence of cardiac and vessels calcification in patients with chronic kidney disease. Isolated calcification of the aortic valve prevailed in the 1st group, combined calcification of the aortic and mitral valves - in the 2nd group. According to the multifactorial stepwise regression analysis there were independent factors of cardiac calcification development: age of the patients (P &lt; 0.001), duration of renal replacement therapy (P = 0.002), value of calcium phosphorus product (P &lt; 0.001), and level of the parathyroid hormone (P = 0.002). According to the multifactorial stepwise regression analysis there were independent factors of vessels calcification: age of the patients (P &lt; 0.001), phosphorus blood level (P &lt; 0.001), and diabetes mellitus (P = 0.005).
